To see another similar layout note being left on a card, look at the foil and non-foil Colos Yearling. The key phrase is "Do A to Do B". I'd probably say that you'd have to kill each head separately - 10 counters on each. Note that you should sort out house rules to cover this - some players use other rules, and these are just my ideas, not official rulings. >5) Millstone type Decks: If you run one "head" out of cards, does that >player just die?
